IS Service Request - Initial Screen

  • This is the initial screen for beginning an IS Service Request, and where the user selects the type of request.
  • New User Setup - is for requesting services for new employees. A job role template can be selected to automatically order all of the services required for that role. For example, the A/P Clerk template will automatically add all the services an A/P clerk will need to perform their job. This helps eliminate ordering incorrectly.
  • Change User - is for changing the services provided to an existing user. This may be to add services or remove services.
  • Change User Name - is for changing the name of a user in all of the systems/services provided.
  • Terminate User - is for notifying all systems to cancel the users account.
  • Application Enhancement - is for requesting modifications to applications or other software development work.
  • Other - is for all other requests that do not fit in a category above.
