Display Order Request - Approval Form

  • When approving a request, a comment form is used to allow the manager to comment on their rational for approving the request.
  • The DIS may elect to have the VP of Sales also review the request by checking the "Require VP Approval" checkbox.
  • The Comment History section displays all of the comments entered in reverse chronological order (newest first).
  • The Spell Check button invokes a spelling checker on the comment memo area.
  • This form is also used for declining a request.
